Overview
Urolithin A (UA) is a gut microbiome-derived postbiotic metabolite produced from ellagitannins and ellagic acid found in pomegranates, berries, and walnuts. However, only approximately 40% of the population harbors the specific gut bacteria capable of producing urolithin A endogenously, making direct supplementation necessary for most individuals. UA's primary mechanism of action is the activation of mitophagy — the selective autophagic removal of dysfunctional mitochondria. By upregulating PINK1/Parkin-mediated mitophagy pathways and AMPK signaling, UA promotes mitochondrial quality control, a process that declines significantly with aging. In a landmark Phase 2 randomized controlled trial (JAMA Network Open, 2022), supplementation with 1000 mg/day of Urolithin A (branded as MitoPure by Amazentis/Timeline) for 4 months in older adults (65-90 years) significantly improved muscle endurance (6-minute walk test), biomarkers of mitochondrial health, and reduced plasma acylcarnitines — markers of incomplete fatty acid oxidation. Additional human studies demonstrated improved mitochondrial gene expression in skeletal muscle biopsies and enhanced cellular NAD+ levels. The compound has received GRAS status from the FDA and is the first postbiotic with robust clinical mitophagy data in humans.
