Pregnenolone for Neurosteroid & Cognitive Wellness — Brain

Master neurosteroid precursor that enhances memory, mood, and neuroprotection through downstream conversion to DHEA, progesterone, and cortisol.

Overview

Pregnenolone is the master neurosteroid precursor synthesized from cholesterol, serving as the starting point for all steroid hormone biosynthesis including DHEA, progesterone, cortisol, testosterone, and estrogen. In the brain, pregnenolone and its sulfated form (pregnenolone sulfate) act as potent neuroactive steroids that modulate GABA-A, NMDA, and sigma-1 receptors, enhancing memory, mood, and neuroprotection. Marx et al. (Psychoneuroendocrinology, 2014, PMID: 24813425) demonstrated that pregnenolone supplementation (500mg/day) significantly reduced PTSD symptom severity in a randomized controlled trial of Iraq/Afghanistan veterans. Pregnenolone levels decline with age, and low levels are associated with depression, anxiety, and cognitive impairment. Ritsner et al. (J Clin Psychiatry, 2010) showed pregnenolone improved cognitive symptoms in schizophrenia patients. Available OTC as a dietary supplement in the US. Generally well-tolerated at doses of 10-100mg daily, though higher doses (>100mg) may cause irritability, anxiety, or hormonal imbalance due to downstream steroid conversion.

Indications

  • Age-related cognitive decline and memory support
  • Mood optimization and neurosteroid balance
  • PTSD symptom management (adjunctive, PMID: 24813425)
  • Neuroprotection and brain health optimization

Mechanism of Action

Pregnenolone synthesis from cholesterol declines with age, reducing availability of neuroprotective steroids in the brain

Dosing

CompoundDoseFrequencyNotes
Pregnenolone10-30 mgOnce daily (morning)Start low to assess individual conversion patterns; sublingual may enhance bioavailability
Pregnenolone50-100 mgOnce daily (morning)Standard dose for cognitive and mood enhancement; most common clinical dose
Pregnenolone100-500 mgOnce dailyHigher dose used in PTSD RCT (PMID: 24813425); requires medical supervision

Safety & Contraindications

  • OTC dietary supplement in the US; generally well-tolerated at 10-100mg daily
  • Converts to multiple downstream hormones - may elevate progesterone, DHEA, or cortisol unpredictably
  • Higher doses (>100mg) may cause irritability, anxiety, insomnia, or acne
  • Contraindicated in hormone-sensitive cancers (breast, prostate, uterine)
  • Monitor hormone levels if using concurrently with HRT or other hormonal therapies
  • May interact with benzodiazepines and other GABAergic drugs (GABA-A modulation)
