Nattokinase for Fibrinolytic & Cardiovascular Support — Aging
Serine protease enzyme derived from fermented soybeans (natto) with fibrinolytic, antihypertensive, and anti-atherosclerotic properties.
Overview
Nattokinase (subtilisin NAT, EC 3.4.21.62) is a serine protease enzyme produced by Bacillus subtilis var. natto during the fermentation of soybeans to produce the traditional Japanese food natto. Discovered by Dr. Hiroyuki Sumi in 1987, nattokinase exhibits potent fibrinolytic activity — it directly cleaves cross-linked fibrin and also enhances the body's endogenous fibrinolytic system by converting plasminogen to plasmin and increasing tissue plasminogen activator (tPA) activity while inactivating plasminogen activator inhibitor-1 (PAI-1). In clinical studies, nattokinase supplementation (2,000-7,000 FU/day) has demonstrated reductions in blood pressure (systolic -5.5 mmHg in a meta-analysis of 11 RCTs), decreased D-dimer and fibrinogen levels, and potential anti-atherosclerotic effects. The NATTOPLAQ trial showed a reduction in carotid intima-media thickness progression in patients at risk for atherosclerosis. Nattokinase survives gastric transit and is absorbed intact into the bloodstream, maintaining fibrinolytic activity for 4-8 hours after oral administration. It is widely used in Japan and Korea as a cardiovascular supplement and is gaining attention in integrative medicine globally.
Indications
- Moderate evidence: Blood pressure reduction (systolic -3 to -6 mmHg)
- Moderate evidence: Fibrinolytic activity (fibrin degradation, D-dimer reduction)
- Emerging evidence: Carotid atherosclerosis progression reduction (NATTOPLAQ)
- Emerging evidence: DVT prophylaxis during long-haul air travel
- Traditional use: Cardiovascular health maintenance (Japan, Korea)
Mechanism of Action
Nattokinase directly cleaves cross-linked fibrin polymers in thrombi, dissolving clots through enzymatic hydrolysis of fibrin at specific peptide bonds
Dosing
| Compound | Dose | Frequency | Notes |
|---|---|---|---|
| Nattokinase | 2,000 FU (100 mg) | Once daily | Standard cardiovascular support dose |
| Nattokinase | 4,000 FU (200 mg) | Once daily | Higher dose for fibrinolytic effect; used in clinical studies |
| Nattokinase | 7,000 FU (350 mg) | Once daily | Maximum studied dose; enhanced blood pressure reduction |
Safety & Contraindications
- May increase bleeding risk — contraindicated with anticoagulants (warfarin, DOACs), antiplatelets
- Discontinue 2 weeks before surgery or invasive procedures
- Soy allergy is a contraindication — derived from fermented soybeans
- Vitamin K2 content in natto food may interfere with warfarin; purified nattokinase supplements are K2-free
- Monitor for signs of bleeding (bruising, epistaxis, dark stools)
- Not recommended in pregnancy or active bleeding disorders
