Overview
Ketoconazole is a broad-spectrum imidazole antifungal that has gained significant attention in dermatology for its anti-androgenic properties when used topically for androgenetic alopecia (AGA). Beyond its antifungal activity, ketoconazole inhibits the cytochrome P450 enzyme CYP17A1 (17-alpha-hydroxylase/17,20-lyase), reducing local testosterone and dihydrotestosterone (DHT) production in the scalp. It also competes with DHT for androgen receptor binding, providing a dual anti-androgenic mechanism directly at the hair follicle. A pivotal study comparing 2% ketoconazole shampoo to 2% minoxidil in men with AGA found comparable improvements in hair density and hair shaft diameter after 6 months. Additional studies demonstrate reductions in scalp sebum production and inflammation — both contributing factors to AGA progression. Ketoconazole also treats seborrheic dermatitis and dandruff (Malassezia overgrowth), conditions that frequently coexist with AGA and can exacerbate hair shedding. The 1% formulation is available OTC (Nizoral A-D), while 2% requires a prescription. It is commonly used as an adjunctive therapy alongside finasteride and/or minoxidil in multi-modal AGA treatment protocols.

Safety & Contraindications
