Glucosamine sulfate is an amino sugar naturally present in cartilage and synovial fluid that serves as a precursor for glycosaminoglycan and proteoglycan synthesis. The crystalline glucosamine sulfate formulation (Rottapharm/MEDA) has the strongest evidence base, with long-term RCTs demonstrating both symptom improvement and structure modification (reduced joint space narrowing) in knee osteoarthritis. ESCEO guidelines recommend it as a first-line pharmacological treatment for knee OA. Results vary significantly by formulation quality.
