Overview
Ginkgo biloba extract (EGb 761) is standardized to 24% flavone glycosides and 6% terpene lactones (ginkgolides and bilobalide). It improves cerebral and peripheral blood flow through vasodilation, reduces blood viscosity, and provides antioxidant protection. Clinical evidence supports modest cognitive benefits in age-related cognitive decline and vascular dementia, though the large GEM trial found no benefit for dementia prevention in healthy elderly. European regulatory agencies approve Ginkgo for cognitive impairment and intermittent claudication.

Mechanism of Action
Ginkgolides (especially ginkgolide B) inhibit platelet-activating factor, reducing platelet aggregation and inflammation

Safety & Contraindications
- Increases bleeding risk; discontinue 2 weeks before surgery
- Avoid combining with anticoagulants, antiplatelet agents, or NSAIDs
- Headache, GI upset, and dizziness are uncommon but reported
- Avoid unprocessed ginkgo seeds (contain ginkgotoxin, a neurotoxin)
- Cycle 8-12 weeks on, then reassess
