Overview
Clomiphene citrate is a first-generation selective estrogen receptor modulator (SERM) comprising a mixture of two stereoisomers: zuclomiphene (cis-isomer, estrogenic) and enclomiphene (trans-isomer, anti-estrogenic). FDA-approved for female ovulatory dysfunction since 1967, clomiphene has been used off-label for decades to treat male hypogonadism while preserving fertility — a critical advantage over exogenous testosterone which suppresses spermatogenesis. By blocking estrogen receptors in the hypothalamus, clomiphene releases negative feedback, increasing pulsatile GnRH secretion and subsequently elevating LH and FSH. Multiple studies demonstrate that clomiphene 25-50 mg daily increases testosterone levels by 100-200% in hypogonadal men. A meta-analysis of 11 studies showed mean testosterone increase from 228 to 487 ng/dL. However, the estrogenic zuclomiphene isomer accumulates over time (half-life ~30 days vs 10 hours for enclomiphene), which may cause estrogenic side effects with chronic use including visual disturbances, mood changes, and reduced efficacy.

Safety & Contraindications
- Visual disturbances (blurred vision, scotomata) — discontinue if occurs; usually reversible
- Zuclomiphene accumulation with chronic use may cause estrogenic effects
- Mood changes, irritability, and emotional lability reported
- Headache, hot flashes, and GI discomfort common
- May increase risk of testicular tumors with very prolonged use (theoretical)
