Overview

Berberine is an isoquinoline alkaloid found in several medicinal plants including Berberis vulgaris (barberry), Coptis chinensis (goldthread), and Hydrastis canadensis (goldenseal). It has been used in traditional Chinese and Ayurvedic medicine for centuries. Modern pharmacological research has revealed that berberine activates AMP-activated protein kinase (AMPK) — the same cellular energy sensor targeted by metformin — producing similar metabolic effects. A landmark meta-analysis of 27 RCTs found that berberine reduced HbA1c by 0.5-0.9%, fasting glucose by 15-25 mg/dL, and LDL cholesterol by 20-30 mg/dL in patients with type 2 diabetes and dyslipidemia. In one frequently cited RCT (Yin et al., 2008), berberine 500 mg TID was as effective as metformin 500 mg TID for glycemic control over 13 weeks. Berberine also modulates gut microbiome composition, reduces intestinal cholesterol absorption, and has anti-inflammatory properties. A significant limitation is poor oral bioavailability (approximately 5%) due to extensive first-pass hepatic and intestinal metabolism by CYP enzymes and P-glycoprotein efflux. GI side effects (diarrhea, constipation, flatulence) are common. Berberine has significant drug interaction potential — it inhibits CYP2D6, CYP2C9, CYP3A4, and P-glycoprotein, potentially increasing levels of many drugs.
