Azelaic acid is an FDA-approved dicarboxylic acid with anti-keratinizing, antibacterial, and tyrosinase-inhibiting properties effective for acne vulgaris, rosacea, and hyperpigmentation disorders.

Overview

Azelaic acid (nonanedioic acid) is a naturally occurring C9 dicarboxylic acid produced by Malassezia furfur on human skin. It is FDA-approved in two formulations: Finacea 15% gel for rosacea and Azelex 20% cream for acne vulgaris, making it one of the few cosmeceutical compounds with full regulatory approval. Over 160 peer-reviewed publications document its triple mechanism of action: (1) anti-keratinizing activity that normalizes follicular hyperkeratinization, (2) selective antibacterial activity against Cutibacterium acnes (formerly Propionibacterium acnes) and Staphylococcus epidermidis without inducing resistance, and (3) reversible competitive inhibition of tyrosinase for treating hyperpigmentation. Unlike hydroquinone, azelaic acid is safe for long-term use and does not cause ochronosis. It is pregnancy category B, making it one of the safest treatment options for acne and hyperpigmentation during pregnancy. Its anti-inflammatory effects include inhibition of thioredoxin reductase and reduction of reactive oxygen species generated by neutrophils.
