Overview

Ayahuasca is a brew of Psychotria viridis (containing DMT) and Banisteriopsis caapi vine (containing beta-carboline MAOIs including harmine and harmaline). The MAOIs prevent oral metabolism of DMT, producing a 4-6 hour psychedelic experience. DMT (N,N-dimethyltryptamine) is a potent endogenous and exogenous 5-HT2A agonist. The first open-label clinical trial (NEJM Evidence 2023, n=30, Brazil) found a single ayahuasca session produced rapid, significant antidepressant effects persisting 7 days post-session — comparable to esketamine. The Imperial College London ayahuasca ceremony study showed reduced psychological inflexibility and increased ego dissolution score correlated with antidepressant response. Standalone vaporized DMT ('machine elves' breakthrough experience, 5-10 minutes) and 'extended-state DMT' protocols (IV infusion for prolonged 30-60 minute experiences) are being researched by Beckley Psytech and Small Pharma. Traditional ceremonial use in South America (Brazil, Peru) is legal. First ayahuasca RCT published in JAMA Global Health 2024.

Mechanism of Action

Beta-carboline alkaloids in B. caapi inhibit monoamine oxidase (MAO-A), preventing first-pass metabolism of DMT in the gut and liver — enabling oral bioavailability and prolonging the DMT effect from 5-10 minutes to 4-6 hours

Safety & Contraindications

  • CRITICAL: Harmaline MAOIs in ayahuasca cause dangerous serotonin syndrome with SSRIs, SNRIs, antidepressants, tramadol, MDMA — washout required (minimum 2 weeks for SSRIs; 5 weeks for fluoxetine)
  • Strict tyramine-restricted diet required before and during (avoid aged cheese, cured meats, fermented foods) — hypertensive crisis risk
  • Cardiovascular monitoring essential — transient hypertension and tachycardia during session
  • Contraindications: cardiac disease, hypertension, psychiatric disorders with psychosis risk, kidney/liver disease
  • Ceremonial setting quality varies enormously — choose experienced, reputable facilitators; 'synthetic' or synthetic harmaline combos not traditional
