Overview

Aniracetam (1-p-anisoyl-2-pyrrolidinone) is a fat-soluble racetam nootropic developed by Hoffmann-La Roche in the 1970s. Unlike piracetam, which is water-soluble, aniracetam's lipophilicity enables faster blood-brain barrier penetration and higher CNS bioavailability. Aniracetam is approved in some European countries (Italy, France) and in Japan (as Draganon/Ampamet) for cognitive impairment, Alzheimer's disease, and post-stroke cognitive decline. It is not FDA-approved in the United States. Aniracetam acts as a positive allosteric modulator of AMPA receptors (an 'ampakine'), enhancing glutamatergic neurotransmission and long-term potentiation (LTP) — the cellular basis of learning and memory. Uniquely among racetams, aniracetam also modulates cholinergic, dopaminergic, and serotonergic neurotransmission, contributing to its distinctive anxiolytic and mood-enhancing properties. Clinical studies in elderly patients with mild to moderate cognitive impairment showed improvements in attention, memory, and emotional stability. The anxiolytic effects are mediated through modulation of serotonin 5-HT2A receptors and dopamine D2/D3 receptors. Aniracetam has a short half-life (1-2.5 hours) requiring multiple daily doses and must be taken with fat-containing food for absorption.
