Overview
7-Keto DHEA (3-acetyl-7-oxo-dehydroepiandrosterone) is a naturally occurring metabolite of dehydroepiandrosterone (DHEA) produced by CYP7B1-mediated hydroxylation. Unlike its parent compound DHEA, 7-Keto cannot be converted back into androstenedione, testosterone, or estrogens because the 7-oxo group prevents the enzymatic conversions required for sex steroid synthesis. This makes 7-Keto DHEA attractive for patients who want the metabolic benefits associated with DHEA without the risk of unwanted androgenic or estrogenic effects. The primary clinical interest in 7-Keto DHEA centers on its thermogenic properties — it increases the activity of three thermogenic enzymes in the liver (glycerol-3-phosphate dehydrogenase, malic enzyme, and fatty acyl-CoA oxidase), potentially increasing resting metabolic rate (RMR). In a small double-blind RCT, overweight adults receiving 200 mg/day of 7-Keto DHEA combined with exercise lost significantly more body weight (6.3 vs 2.2 lbs) and body fat percentage compared to placebo over 8 weeks. However, the evidence base is limited to a few small studies from a single research group, and independent replication is needed. 7-Keto DHEA also shows immunoenhancing properties in preclinical studies, with potential to reverse age-related immune decline.
